My feeble old ivy grew vigorously last summer (on an east facing fence). Now it looks dreadful. Shall I cut it back? Hard? or what do you advise?

The quick answer is, that it is too soon to tell. This year’s cold winter wind caused foliage injury to many exposed evergreen plants including English Ivy (Hedera helix).
I would suggest waiting until it starts to regrow from its dormant buds. Since it thrived last summer, I would imagine you have good soil and the right conditions for its growth. You will soon see it growing again. However, anything that has not developed by June, can be cut back to a healthy bud then.
Patience is sometimes the gardeners best friend.
